12-lead ECG and STEMI MasterClass

Instructional Hours: 4.00

Learning Objectives

  • Describe coronary thrombus formation and the significance of platelet clot versus a fibrin clot
  • Predict thrombus type based on ECG findings
  • Correctly apply current definitions of ST elevation and correctly evaluate ECG for presence of STEMI criteria
  • Reduce ECG artifact through skin prep
  • Locate the posterior wall of the left ventricle
  • Recognize indication for and process to acquire posterior ECG leads
  • Apply ST elevation criteria to posterior leads
  • Discuss the physiologic rationale for the dynamic changes of the ST segment during STEMI
  • Describe the importance of Serial ECGs to increase correct STEMI identification
  • Describe the importance of obtaining early ECGs to increase correct STEMI identification
  • Explain the difference between ischemia, injury and infarct based on ECG findings
  • Describe the anticipated evolutionary waveform changes indicative of STEMI
  • Describe the anticipated ECG patterns of reciprocal changes form STEMI
  • Define STEMI impostor
  • Recall the most commonly encountered STEMI impostors
  • Analyze ECG to identify or rule out voltage criteria for LVH
  • Analyze ECG to rule out BBB and ventricular rhythms
  • Analyze ECG to functionally rule out early repolarization and pericarditis as cause of ST elevation
  • Evaluate clinical presentation to functionally rule out early repolarization as likely causes of ST elevation
  • Categorize ECG findings as either: STEMI likely, STEMI unlikely or Consider STEMI
  • Express the complexities of evaluating the possibility of STEMI in the presence of a STEMI impostor
  • Formulate a STEMI alert decision by integrating the patient's clinical presentation, clinician's assessment of the ECG findings and specific information from the device
